Having trouble logging in? Make sure you have typed your user name correctly. Only authorized users that have been assigned a user name and password may login to the system. Please read How to Become an Authorized User. Having trouble remembering your password? Each user can create a self-help hint for their password on the "Your Defaults" screen. To have the self-help hint sent to the email address you entered there, answer the following question correctly. What is your User Name? What is your mother's maiden name?

Do you still need help logging in? Contact the person(s) in your office that establishes the employee and clinic defaults for your practice in the IMPACT SIIS system. This person has the highest security level of "Key-Master." The Key-Master can give you a new temporary password to login with. Once you login with the temporary password, be sure to go to the "Your Defaults" screen and give yourself a new permanent password and self-help hint. If you are the Key-Master at your practice and cannot remember your password contact the Ohio Department of Health. (Note: Each practice should give two people the "Key-Master" security level for this circumstance.)
